I created a screen cast to show how to insert video into a panorama and have that video be aligned.

Using PTGui, VideoStitch, Premiere, and Pano2VR
Steps:
Shoot and stitch a panorama
Shoot a video at the same time. For simplicity use the same lens. Can use a slightly longer. Because most cameras can capture stills at a much higher resolution than video.
Add video to VideoStitch.
VideoStitch: New Calibration -> Launches PTGui
PTGui: Add Pano, new lens, set lens distortion to 0
PTGui: Add Control points, optimize.
PTGui: Set out put to recliner, Adjust View, Use Numeric Transform to point of view, remember offset, and Field of View used.
PTGui: set size to a factor of 4
PTGui: save.
VideoStitch: Apply Calibration
VideoStitch: Render
Pano2VR: Media | Modify | Double click to add
Pano2VR: set FoV, save and close
In text editor modify the Pan and tilt values to match the values used to offset pano to center video. Save.
Pano2VR: reopen project, Export pano.
