1. Operating system and browser name and version.
2. What step in the work flow were you doing JUST BEFORE it failed
3. what software you installed and operating systems updates occurred on your system 1 week up to the date this started

There is a closed bug report program crash when using the patch tool, if its the same thing then its fixed in next release.
But when reporting bugs its best to have a look at our FAQ Page.
For us the best things are which version, platform and steps to reproduce the problem.
